Clinical Coordinator Pharmacy


Primary Duties and Responsibilities
  Determines the criteria for conducting effective clinical rounds taking into account current pharmacy initiatives such as anticoagulation management, pain management and medication reconciliation. Coordinates the scheduling of pharmacists to conduct clinical rounds. Prepares the agenda for the Pharmacy and Therapeutics committee.  Subject matter is presented with confidence in a clear and concise manner. Prepares drug utilization reviews to insure drugs are being used appropriately according to indications and in a cost effective manner.  Culture and sensitivity results are used to determine appropriate use of antibiotics. Oversees the daily operation of the sterile products lab.  Enforces USP 797 standards. Reviews adverse drug reaction reports submitted to pharmacy and copies of orders with "trigger antidotes" submitted by pharmacists.  Analyzes data and reports findings to Pharmacy and Therapeutics committee. Maintains the hospital drug formulary system.  Adds new drugs as approved by the P&T committee, deletes those no longer used.  Advises director and staff of changes. Conducts annual review of formulary.  Reports findings to P&T committee. Performs staff pharmacist functions and order entry when staffing shortages occur due to sickness, vacations or resignations.  Works evenings and weekends as required. Meets with drug manufacturer representatives to discuss the merits of new medications or medication delivery systems. Provides informative articles for the Pharmacy newsletter. Serves on various hospital committees as designated by the Director of Pharmacy such as Infection Control, Critical Care, Psychiatry, and Medication Safety. Maintains and updates Pharmacy library to provide current drug information to fellow pharmacists and other health professionals. Maintains performance improvement programs (i.e., IV to PO conversion, antibiotic prophylaxis, anticoagulation, etc.) Implements quality improvement initiatives, makes suggestions to the director. Collects and maintains statistics on performance improvement, quality assurance, and quality control programs.  Serve as a knowledgeable drug information resource for the institutional staff and respective physicians. Develops the skills of others in the department.  May be responsible for the orientation of new personnel or assisting with the internship of a pharmacy apprentice. Keeps abreast of current developments in the pharmaceutical industry regarding new drugs and new technologies. Review patients' charts for drug audits, drug usage evaluation, and other studies required by the medical staff or accrediting agencies. Explores innovative methods of increasing pharmacy's value to the institution; develops and implements these services; i.e., pain management, anticoagulation, medication error reduction, etc. Defines and monitors indicators of quality assurance necessary for quality patient care and to meet or exceed standards of accrediting agencies. Conducts the preceptorship of pharmacy student externs assigned to this institution. Improves or implements processes to meet Joint Commission standards.  Educates staff pharmacists. Conducts medical record review for long term care unit (2 South).  Prepares quarterly report. Performs other duties as required.     EDUCATION/EXPERIENCE REQUIREMENTS: Bachelor of Science in Pharmacy required; Doctorate of Pharmacy preferred 5 years of experience required in a hospital environment Maryland State License. 5545

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
