Clinical Pharmacist

Job Summary Assists the clinical coordinator with providing clinical services to our patients. Supervises the scheduling and conduction of clinical rounds functions as required. Minimum Qualifications Education/Experience Doctorate of Pharmacy preferred; Bachelor of Science in Pharmacy accepted PGY1 Residency preferred 3-5 years hospital experience preferred Supervisory experience License/Certification/Registration Maryland State License Knowledge, Skills & Abilities Ability to comprehend and follow the policies and procedures for Southern Maryland Hospital Center. Ability to read, write and speak or communicate in English to successfully accomplish the essential duties of the position. Ability to demonstrate ethical behavior that supports the hospital's mission, values and commitment to compliance with all federal, state and regulatory laws. Inspires trust and exhibits honesty and integrity within the scope of daily activities. Exhibits professionalism, courtesy and excellent customer service, while interacting with patients, guests and co-workers. Ability to conduct retrospective medical record review for drug use evaluation, adverse drug reactions and quality assurance measures. Knowledge of disease state management in such areas as anticoagulation, nutrition, and pain management. Ability to interpret lab values and suggest changes in medication therapy. Knowledge of clean room principles and USP 797 standards for compound sterile products. Ability to demonstrate interpersonal skills with patients, coworkers and others demonstrating excellent customer service skills. Basic knowledge of hospital pharmacy practice with the ability to demonstrate competence in the following areas:

//www.neevia.com Anticoagulation management Adverse drug reaction reporting Conversion of patients from IV to oral therapy Medication reconciliation Maryland pharmacy law Primary Duties and Responsibilities Assists the clinical coordinator with medical record review and the collection of data for anticoagulation management, adverse drug reaction reporting and drug use evaluation. Performs medical record review for Subacute Care to meet state requirements for long-term care, as assigned Assists with the preparation of urgently needed or complex IV products. Participates in quality assurance initiatives within department. Oversees the preparation of IV chemotherapy by technicians. Coordinates with nursing the timeline from preparation to administration. Orders and prepares products as needed. Functions as staff pharmacist when coverage is needed due to sickness, vacations or staff vacancies. Works various shifts, day or evening and alternate weekends. On rare occasions, may cover night shift PTO. Oversees the accuracy and efficiency of the work performed by the Staff Pharmacist, Pharmacy Technicians. Participates in Regulatory Activities within the department and the hospital to ensure compliance. Coaches and mentors pharmacy staff in the application of clinical knowledge Participates in Quality Improvement initiatives and metrics development and reporting Performs other duties as required. Salary Range:
$80K -- $100K
